- Useful to aid early diagnosis and monitor disease activity.
- Periarticular high signal at the sacroiliac joints on STIR (i.e. oedema), marrow/sacroiliac (SI) joint enhancement with gadolinium.
- The hyperaemia and oedema at the anterior corners of the vertebral body cause high T2 signal— Romanus lesions .
- Look for fractures (low signal on T1, high signal on T2/STIR); these commonly occur horizontally through the intervertebral disc and may cause disc herniation and cord compression.
