This is an uncommon variant of HCC affecting young patients with no risk factors. The tumour is typically large at diagnosis. The overall prognosis is better than HCC, with a 5-year mortality rate of 30%-40% (90% with HCC). Local recurrence and nodal metastases are common.
US

•  Solid, encapsulated mass of 5-20 cm at presentation
•  Calcification
•  Hyperechoic scar (60%)

CT
•  Lobulated margin.
•  Hypodense, particularly the central scar.
•  Punctate calcification in 50%.
•  Moderate, peripheral arterial enhancement post-contrast.
•  Low attenuation on the portal phase (i.e. rapid washout).
•  Scar does not enhance, but often contains calcification.

MRI

•  Mass low signal on T1, high on T2

•  Central scar is T2 hypo-intense
