This is AVN of the capitellum (‘little leaguer’s elbow’) and is more common in athletic boys aged 5-10 years. Thought to be due to microtrauma.
PLAIN FILM
• Radiolucent band may appear at the capitellum, similar to hip osteonecrosis.
• Irregular contour.
• Increased density.
• Widening of the radiohumeral space.
MRI
• Low signal on T1, high signal on T2.
• No loose body (otherwise osteochondritis dissecans should be considered)
