Due to a failure in fusion of the dorsal and ventral ducts. The major papilla drains the ventral duct (head) and a minor papilla drains the dorsal duct (body and tail). It may cause a functional stenosis and pancreatitis and is associated with an increased risk of pancreatic cancer.
CT

•  Lobulation/enlargement of the pancreatic head
•  Separation of the ventral and dorsal portion of the pancreas by a cleft of fat
•  Actual visualisation of the divided ducts

MRCP

•  Long, narrow dorsal duct that is dominant (drains most of the pancreas) and empties via the minor papilla—this duct may be dilated due to stenosis at the minor papilla.
•  Short ventral duct empties to the major papilla.
•  ‘Crossing duct sign’—best seen on coronal MIPs.
