This is a serious complication of radiotherapy. It is dose related and may be progressive and fatal. It tends to occur between 1 and 10 years post-treatment.
CT/MRI
• Lesion with mass effect and rim enhancement at the focus of previous radiotherapy.
• Can look very similar to tumour recurrence—however, it regresses over time, is PET negative and tumour recurrence shows a choline peak on MRI spectroscopy.
• Large vessels included in the radiotherapy field may stricture or there may be telangiectasia mimicking an AVM.
