This is the most common cause of optic nerve enlargement and is mostly found in children less than 10 years of age (80%  less than  20 years of age). It is more common in patients with NF1 and may be bilateral.
MRI

•  Mild high signal on T2 with variable (poor patchy) enhancement.
•  The nerve is kinked/buckled, sausage-shaped or fusiform.
•  Widens optic canal if there is intracranial extension.
