This is a benign lesion consisting of perineural fibrosis that entraps a plantar digital nerve; 80% of those affected are women. There is a strong predilection for the third common digital nerve. It presents with foot pain exacerbated by walking.
US
• Hypoechoic, rounded lesion found between the metatarsals.
• Larger in the axial than sagittal plane.
• Treated effectively with a steroid/local anaesthetic injection in 75% at 1 week’s follow-up.
MRI
• Intermediate T1 signal (i.e. isointense to muscle) with enhancement post-gadolinium.
• Low T2 signal.
• It is often associated with metatarsal bursitis, which is high signal on STIR.
