Chronic reflux causes metaplasia of the normal squamous epithelium so it becomes columnar. Diagnosis is made by endoscopy and biopsy; >2 cm of metaplasia is diagnostic.
Barrett oesophagus is linked to a 40-fold increased risk of oesophageal cancer.
The immediate precursor is high-grade dysplasia.
BARIUM SWALLOW
• Classic appearance is a wide, ‘patulous’ hiatal segment with an accompanying hiatus hernia.• Long stricture in the mid or lower oesophagus.• Large, deep, solitary ulcer (Barrett ulcer).• Fine reticular pattern.• Thickened, irregular mucosal folds.• Fine granular mucosal pattern.
