Legg-Calve-Perthes disease is a common idiopathic AVN of the capital femoral epiphysis that is more common in boys between 4 and 8 years of age. It is bilateral in up to 20%.
PLAIN FILM 
•  Subchondral lucency is the earliest sign; the ‘crescent sign’.
•  In early disease, also look for widening of the medial joint space, a small femoral epiphysis, sclerosis of the femoral head and localised bone demineralisation.
•  In late disease, there is fragmentation of the femoral head, subchondral fracture on the anterolateral epiphysis and cysts in the femoral neck.

•  Staging:
  • –  1—normal radiograph
  • –  2—sclerosis and cyst formation
  • –  3—loss of structural integrity
  • –  4—acetabular involvement
NUCLEAR MEDICINE

•  Reduced uptake in early disease.
•  Uptake is increased in late disease.
