Primary biliary cirrhosis is the inflammatory destruction of peripheral bile ducts leading to cirrhosis. In total, 90% of patients are female. It is associated with autoimmune disorders including rheumatoid arthritis, Sjogren syndrome, scleroderma and Hashimoto thyroiditis. The disease is limited to the intrahepatic bile ducts.

US
• Cirrhosis—check for areas of atrophy, compensatory hypertrophy (segment I) and regenerative nodules.
• Peripheral duct abnormalities in advanced disease.
CT
• Dilated intrahepatic ducts that do not communicate with the main ducts.
• Hyperattenuating, hypertrophied caudate lobe.
• Surrounded by hypoattenuating, rind-like right lobe (pseudotumour).
• Shrunken left lobe.
