Refers to a common defect in the pars interarticularis causing lower back pain in about 25% of cases. A total of 50% of cases develop spondylolisthesis. It is thought to arise from repeated microfractures and elongation of a congenitally weakened pars. The pars defect is usually held together by fibrocartilage and may heal (which may account for a unilateral pars defect).
PLAIN FILM
• Visible abnormality in late childhood/adolescence.
• Scottie dog appearance on oblique radiographs (the dog’s collar is the defect).
• 95% occur at L5.
CT
• Defect in the pars interarticularis—most clearly demonstrated on sagittal reconstructions.
