Unusual diagnosis most often presenting with localized back pain. It is associated with diabetes, IVDU, trauma, surgery, immunosuppression and renal failure. Most is seeded haematogeneously; direct spread is usually from vertebral osteomyelitis or psoas abscess.
MRI
• Intermediate T1 signal in the epidural space posterior to the vertebral body (T1 signal is iso-slightly hyperintense to the adjacent cord).
• Focal T2 hyperintense epidural signal, which is difficult to separate from cerebrospinal fluid (CSF).
• The solid component enhances following intravenous gadolinium.
• Check for cord compression.
• Bone marrow/soft tissue oedema on fat-suppressed images.
