This is a common complication of ankle sprains, but may also result from an inflammatory arthropathy. The sinus tarsi is the lateral space between the talus and calcaneus containing fat, nerves and the talocalcaneal interosseous ligaments. Ligamentous injury in the sinus tarsi causes lateral foot pain and hind-foot instability that typically presents between ages 30 and 40 years.
MRI
• Alteration of the fat signal (e.g. to low T1 and T2) in the sinus is the most specific sign (replaced with either fluid or scar tissue).
• Subtalar joint osteoarthritis.
• Synovial thickening.
• Diffuse enhancement of the tarsal sinus post-gadolinium.
