In patients with continued back pain post-discectomy/laminectomy, the cause may be either post-operative epidural fibrosis (i.e. scar tissue) or recurrent disc herniation.
MRI
• Pre- and post-contrast T1 imaging
• Scar: epidural soft tissue at the site of surgery, irregular borders, early homogeneous enhancement and thecal retraction towards the soft tissue mass
• Disc: smooth borders and no central enhancement/delayed enhancement
