SPIO and gadolinium are useful for the detection of hepatocellular carcinoma in cirrhotic patients in whom the detection is difficult with gadolinium alone.
• SPIO is taken up by hepatocytes and causes marked T2 shortening (i.e. signal loss).
• The lesion composed of abnormal cells does not take up SPIO and therefore appears high signal.
