Considerable overlap occurs in the CT features of Mycobacterium tuberculosis and Mycobacterium avium-intracellulare : however, Mycobacterium avium-intracellulare usually occurs at a greater degree of immunosuppression when the CD4 count falls below 50-100 cells/pL.

• Cardinal imaging features are lymph node enlargement, hepatosplenomegaly and focal lesions in the liver (Mycobacterium tuberculosis more), spleen (Mycobacterium tuberculosis more) or kidneys.
• Fibrotic fixed peritonitis less common—look for large omental masses, matted and tethered bowel loops and mesentery and loculated ascites.
• Proximal small bowel thickening is a feature of MAI and can resemble Whipple disease.
