Mostly bulbar (95%) and on the anterior wall. They are a common cause of perforation. Healed ulcers may cause duodenal stenosis.
PLAIN FILM
• Check for pneumoperitoneum
• Subdiaphragmatic free gas, Rigler sign, outlining of the falciform ligament
• Fat stranding, free gas locules and fluid adjacent to the proximal duodenum/gallbladder.
• Check for free gas at the margin of the anterior liver.
• Posterior duodenal wall ulcers may invade the pancreas.
