Causes are either intrahepatic (e.g. cirrhosis, tumour invasion or hypercoagulability) or extrahepatic (e.g. post-surgical, tumour invasion, sepsis or clot propagation). CT/MRI are useful if US is non-diagnostic.
US
• Echogenic material in the portal vein—if the material has vascularity, then it is likely to be tumour.
• Portal vein diameter >15 mm.
• No flow in the portal vein.
• Ascites.
• Splenomegaly.
