Correct answers :

3. What is the dreaded complication when present in this location?

Fibrosing Mediastinitis
Non malignant fibrous tissue proliferative condition occurring within the mediastinum.
Key Features
• It’s classically caused by histoplasmosis (but the most common cause is actually idiopathic).
• Other causes include TB, radiation, and Sarcoid.
• It’s a soft tissue mass with calcifications that infiltrates the normal fat planes.
• It has been known to cause superior vena cava syndrome.
• It’s associated with retroperitoneal fibrosis when idiopathic – in the “IgG4” spectrum / associated disorders.
• IgG4s that go together (if you see one think of the others… the test writers will):
• Fibrosing Mediastinitis
• Retroperitoneal Fibrosis
• Autoimmune Pancreatitis
• Tubulointerstitial nephritis
• Orbital Pseudotumor
• Basically all the weird fibrosing shit
