An X-linked disease (therefore only males are affected) due to a deficiency of either factor 8 or 9. Haemophilia arthopathy arises from repeated bleeds, leading to the formation of pannus that erodes cartilage and leads to loss of subchondral bone and subarticular cyst formation. Intra-articular haemorrhage causes a chronic synovitis, synovial proliferation and periarticular osteopenia. Typically presents in affected patients aged 10-20 years with a tense, warm joint, fever and leucocytosis.
PLAIN FILM
•  Accelerated maturation and enlargement of the epiphysis
•  Soft tissue swelling
•  Juxta-articular osteoporosis
•  Erosion of the articular surface with subchondral cysts
•  Preservation of joint space
Squared patella with widening of the intercondylar notch
•  Medial slanting of the tibiotalar joint and flattening of the condylar surface
MRI
•  Low signal returned from hypertrophied synovium due to magnetic susceptibility of haemosiderin.
•  Articular surfaces are devoid of cartilage.
•  Joint effusion.
