Occurs where the tip of the dens protrudes above the basion-opisthion line (the opening of the foramen magnum, also known as the McRae line). 
Commonly associated with 
rheumatoid arthritis (RA; 10% affected) and platybasia (flattening of the skull base, cranial basal angle >136°), 
as well as rickets, osteomalacia, osteogenesis imperfecta, 
Paget disease, fibrous dysplasia, 
Klippel-Feil syndrome and hyperparathyroidism.
CT
•  Sagittal reformat shows tip of the dens <5 mm below the basion-opisthion line.
