Brain function is normal with isolated callosal agenesis. Partial agenesis always affects the splenium or rostrum (the posterior callosum develops last). In total, 60% have developmental anomalies elsewhere in the body and 50% have other neurological anomalies (e.g. holoprosencephaly [not lobar], Chiari II malformation, Dandy-Walker syndrome, encephalocele, etc.).
US

 Can be diagnosed on the 20-week scan
•  Colpocephaly ( dilated trigones, occipital horns and posterior temporal horns)
•  Absent splenium
•  Dilated and elevated third ventricle

MRI

•  Lateral ventricles have a ‘steerhorn’ appearance on the coronals.
•  Look for an absent cavum septum pellucidum/cingulate gyrus, high-riding third ventricle and colpocephaly.
