Means ‘smooth’, same as agyria (pachygyria = broad, flat gyri). Due to arrested neuronal migration. The brain looks as it should at 18-20 weeks of gestation. It is the most severe malformation of cortical development.
MRI

•  Thickened, smooth cortex with slight indentation at the Sylvian fissures.
•  May be localised to a single lobe and usually the cerebellum is spared.
•  Dilated occipital horns of the lateral ventricles (i.e. colpocephaly).
