Cryptosporidium and CMV are the causes of cholangiopathy (more commonly CMV).
It results in intrahepatic and extrahepatic duct dilatation due to periductal fibrosis.
The presence of papillary stenosis is useful in distinguishing these conditions from primary sclerosing cholangitis.
MRCP
• Irregular strictures of intra- and extra-hepatic ducts with associated duct dilatation
• Papillary stenosis and intraluminal filling defects due to granulomas
• Duct wall thickening
• Acalculous cholecystitis with or without fluid collections
