This is a rare, benign cartilaginous tumour occurring before growth plate closure. Presentation is with a long history of pain. It may become locally aggressive and rarely metastasises.
PLAIN FILM
• Hallmark is a well-defined osteolytic lesion that is centrally or eccentrically located within the epiphysis or apophysis of a long bone.
• Most common in the femur (33%), then humerus and proximal tibia (20% each).
• Thin sclerotic rim.
• Usually internal matrix is chondroid (i.e. rings and arcs) of calcification.
• Useful for assessing extension into the metaphysis.
• Lobulated margin of low signal intensity corresponding to the sclerotic margin on the x-ray.
• Low signal lesion on T1, variable on T2 and high on STIR.
• Lesion may contain fluid-fluid levels.
• Florid marrow oedema.
