Common, mostly from the lungs, breast, melanoma, gastrointestinal tract and renal. Lymphoma (mostly non-Hodgkin) may also spread to the adrenals.
CT
• Small lesions that are difficult to distinguish from benign adenoma; stability over 6 months suggests benignity.
• Expect delayed washout on adrenal CT.
• Large masses with a heterogeneous appearance (e.g. central necrosis, haemorrhage) and irregular borders suggest frank malignancy.
NUCLEAR MEDICINE
• PET/CT is useful for assessing indeterminate lesions—sensitive and specific for differentiating benign from malignant disease.
