Normal ovarian follicles contain cysts. Cysts less than 3 cm are physiological (follicular cyst). Cysts larger than 4 cm are more likely to tort. Best assessed with transvaginal US first.
US
• Thin-walled cyst with no Doppler flow (first half of menstrual cycle).
• Marked peripheral vascularity is seen later in the menstrual cycle, when the cyst may become thickwalled and contain hyperechoic material (haemorrhage; i.e. corpus luteum cyst).
• Indeterminate lesions are followed up at 4-8 weeks when the cyst should have decreased in size (due to different phase of the menstrual cycle).
