From 18 weeks to term, the main change is cortical infolding. An adult pattern of sulcation is reached by 38 weeks. The mainstay of imaging is US at this stage.
In the first year of life, Tl-weighted MRI provides the best view of the myelinating structures—active myelination is high signal on Tl. From 1-1.5 years of age, T2 is better.
• Myelination occurs from caudal to cranial and from posterior to anterior.
• The brain has an adult appearance on T1 at 8 months, and on T2 at 18 months.
