Congenital sagittal division of the spinal cord into two hemichords, each containing a central canal, dorsal horn and ventral horn. Each hemicord has its own blood supply and the hemicords reunite caudally. More common in females.
SPINE X-RAY
• Segmentation anomalies are common, 50%-75% have a scoliosis.
• Look for spina bifida at multiple levels, anteroposterior vertebral body narrowing and fusion/thickening of the laminae.
• cleft is located below T9 in 85% of cases
MRI
• Dual cords (may be of unequal size), most common between T9 and S1.
• In 75%, the conus lies below L2 (i.e. tethered cord).
• About 40% have separate dural sheaths.
• Myelomeningocele is present in up to 30%, syringohydromyelia in 50%.
• Also associated with Chiari II malformations and dermoid cysts.
• tethered cord is seen in more than 50% of cases
