This is an acute autoimmune encephalomyelitis that is usually triggered by an inflammatory response to e.g. a viral infection or a vaccination. It gives rise to multi-focal neurology.
MAGNETIC RESONANCE IMAGING
• Demyelination in a perivenous pattern is a hallmark.
• Multiple, large and confluent/punctate foci of high T2 signal in the subcortical white matter, cerebellum and grey matter.
• The appearance is indistinguishable from multiple sclerosis (MS) (settled with clinical correlation).
• Hurst disease is a haemorrhagic hyperacute variant that causes a marked generalised mass effect and is fatal in a matter of days.
