Congenital or acquired (e.g. tumour). Aqueductal stenosis is the most common cause of congenital hydrocephalus.
MRI
• Hydrocephalus (may be extreme)—very dilated third ventricle with normal fourth ventricle.
• The corpus callosum appears stretched and thinned.
• The aqueduct has a funnel shape on a midline sagittal slice.
• Expect a normal posterior fossa (distinguishes from e.g. Chiari and Dandy-Walker malformations).
• The choroid plexus may be seen suspended in the ventricle (‘dangling choroid sign’).
