Renal artery thrombosis
Rare complication, requires urgent surgical intervention to save the graft.
US
•  No arterial or venous signal on Doppler
Acute tubular necrosis (ATN)

Most common cause of delayed graft function (need for dialysis in the first week post-transplant).
Usually presents in cadaveric grafts. Rare late complication.

•  Usually normal findings and normal Doppler flow
•  May show reversed arterial flow in diastole

Venous thrombosis 
Expect a tender, swollen graft and sudden drop in renal function. Most occur in the first week posttransplant.
US
•  Enlarged with or without hypoechoic graft.
•  Doppler shows no venous flow and arterial flow is reversed in diastole.
Peri-transplant haematoma

Small collections are common after surgery. Problematic if large or pressuring the graft.
US
•  Hypoechoic crescent adjacent to the graft
•  Free fluid in the abdomen with intraperitoneal grafts
