The temporal lobe is the most epileptogenic part of the brain. About two-thirds of those with temporal lobe seizures have hippocampal sclerosis (and atrophy of the amygdala, maxillary bodies and fornix), termed ‘mesial temporal sclerosis’ on neuroimaging.
MRI

•  Thin-section coronal oblique T2 is most useful.
•  Imaging shows hippocampal atrophy (bilateral in up to 20%) and focal hippocampal high signal on FLAIR.
•  Ex vacuo dilatation of the adjacent temporal horn.

NUCLEAR MEDICINE

•  Positron emission tomography (PET) or SPECT performed between seizures (i.e. inter-ictal) show a focus of reduced radiotracer uptake at the seizure focus.
•  Ictal SPECT using hexamethylpropyleneamine oxime will show increased tracer uptake at the seizure focus—rarely used clinically.
