Malformation detected either in utero, with a neonatal pattern (less than 1 month), with an infantile pattern or with an adult pattern (>1 years—low flow). In utero and neonatal manifestations are due to either direct mass effect of the vein of Galen aneurysm (commonly hydrocephalus due to pressure on the aqueduct) or secondary high-output cardiac failure (cardiomegaly).
Three types:
• I: Arteriovenous (AV) fistula
• II: Angiomatous malformation of the basal ganglia, thalalmi and midbrain
• III: Both features
PLAIN FILM
• Chest x-ray shows cardiomegaly.
US
• Midline cystic mass posterior to the third ventricle containing mobile echogenic sparkles
• With or without hydrocephalus
MRI
• Vein of Galen aneurysms may bleed or cause infarction by a steal mechanism.
