A pre-sacral space of less than 15 mm is normal when measured between the mid-rectum and the sacrum. The most common cause of pre-sacral widening is rectal inflammation or malignancy.
Causes (work from the rectum backwards):
• Bowel (inflammatory bowel disease or tumour)
• Fat (pelvic lipomatosis or fibrosis, previous surgery or radiotherapy or developmental cysts: epidermoid, dermoid or duplication)
• Sacral disease (teratoma, meningocele, chordoma, neurofibroma or osteomyelitis)
