This is a serious infection of the external ear canal, almost always due to pseudomonas. It is more common in the immunocompromised, especially diabetics. It erodes the skull base, causing cranial nerve palsies and sigmoid sinus thrombosis.
CT
• Soft tissue mass in external auditory canal, with bone erosion
• Fluid-filled opacification of mastoid air cells
MRI
• Best for early detection of osteomyelitis, granulation tissue is low on T1 and T2.
• Check for sinus thrombosis.
