Also known as otospongiosis, this is the most frequent cause of hearing loss in young adults. Vascular/fibrotic bone changes are the cause of the sclerosis. Subtypes are fenestral (most common 80% , affects the oval window and stapes- sensorineural hearing loss
) and retrofenestral (20%, affects the otic capsule around the cochlear).

• Small bone lucencies seen in early phase either anterior to the oval window (fenestral) or around the cochlear (retrofenestral).
• Findings are subtle and often symmetrical.
