This is an autoimmune disease associated with human leukocyte antigen (HLA) antibodies.
Typically, it presents with thyrotoxicosis, goitre and eye disease (bilateral enlargement of the extraocular muscles).
US
• Enlarged gland, heterogeneous echotexture with spotty hypoechoic appearance
• Marked hyperaemia—‘thyroid inferno’
CT/MRI
• Useful for establishing a diagnosis of thyroid associated ophthalmopathy.
• Enlargement of the extraocular muscles with sparing of the tendon insertions: muscles most commonly affected are inferior > medial > superior > lateral > oblique (mnemonic = ‘I’M SLO’).
• May only involve the retrobulbar fat.
NUCLEAR MEDICINE
• Uniform increased uptake
• Elevated iodine uptake in up to 80%
