This is an autosomal dominant primary form of HOA. It causes a thick, symmetrical periostitis of the distal long bones, typically in young men, which arrests in adulthood. It is painless.
Plain film 
• Symmetrical smooth lamellar periosteal reaction affecting the diametaphyseal regions of the long bones, particularly the dorsal and medial aspects.
Bone scan
• Parallel track’ sign, increased tracer uptake along the cortical margins of the metaphysis and diaphysis of the affected bones.
Hypertrophic osteoarthropathy (HOA). Ankle x-ray demonstrating a lamellar solid periosteal reaction of the tibia and fibula.
