Chronic inflammatory change, usually from an obstructing (staghorn) calculus. Begins within the kidney. When advanced, it may involve the retroperitoneum. Treatment with antibiotics/nephrectomy.
US
• Diffuse renal enlargement with central calculus
• Hydronephrosis/pyonephrosis
• Perinephric collection or soft tissue
• Rim-enhancing hypodense renal masses, central calculus, little or no excretion of contrast.
• Look for aggressive perinephric and retroperitoneal extension.
Post-contrast CT scan of the abdomen showing a large, mixed-density lesion arising from the right kidney. Note a large staghorn calculus on the medial side of the mass. The findings are in keeping with xanthogranulomatous pyelonephritis.
