Rare, aggressive tumour. Presents with dysmenorrhoea. Tends to affect a younger age group than endometrial cancer. Most common subtype is leiomyosarcoma (beware rapidly enlarging fibroid with ill-defined margins).
CT
• Useful for demonstrating lymph node involvement, peritoneal disease and metastases
MRI
• On T1 post-contrast, the mass enhances less than the adjacent myometrium and cervix.
• Heterogeneous appearance to the lesion.
• Large mass with a greater myometrial component than other endometrial cancers.
• Leiomyosarcoma contains foci of haemorrhage and calcification.
