This predominantly affects elderly women and consists of a triad of a rotator cuff tear, osteoarthritis and joint effusion containing crystals (calcium hydroxyapatite and calcium pyrophosphate dehydrate). It results in a dysfunctional shoulder from the rotator cuff injury, is often bilateral (dominant side always involved) and may also affect the knee.
PLAIN FILM
• Superior subluxation of the humerus due to complete cuff tear.
• Osteoarthritis (loss of joint space, subchondral cysts and sclerosis).
• Loose bodies in the joint.
• Look for changes in the lateral compartment when the knee is affected.
Glenohumeral joint arthropathy characterized by joint space narrowing, remodeling of the humeral head and glenoid (with elongation of the articulating surfaces) and synovial calcification.
