• There is skeletal tracer uptake, but little or no tracer uptake in the soft tissues or urinary tract (i.e. absent kidneys).
• The most common cause is diffuse bone metastases (e.g. prostate, breast or lung).
• Metabolic bone disease also common (e.g. renal osteodystrophy; look for lack of radiotracer in the bladder—a classic appearance)
• Other causes: osteomalacia, hyperthyroidism, Paget disease, myeloproliferative disorders, myelofibrosis, lymphoma, mastocytosis, leukaemia
