This is a rare syndrome due to obstruction of the common bile duct owing to extrinsic compression from a large calculus in the cystic duct. Fistulation may occur.

• Dilatation of the common bile duct to the level of a stone that lies outside of the CBD (i.e. in the cystic duct).
