This is an inherited or acquired multi-system disease due to iron deposition (including out of the reticuloendothelial system). The primary form is inherited by autosomal recessive inheritance. There is an increased risk of malignancy in the liver, especially HCC. Iron is also deposited to the skin (90%), myocardium (15%), endocrine glands and joints (20%). 30% of patients are diabetic due to pancreatic damage.
CT
• Hepatosplenomegaly
• Liver attenuation 75-135 HU non-contrast
MRI
• Iron causes signal loss—best seen on T2* gradient echo (also seen on T1).
• Significant signal loss on T2 is characteristic.
• Signal loss in liver and pancreas—the spleen is spared.
