This is the most common hip abnormality affecting adolescents aged 10-16 years. It is more common in boys, obesity, trauma, hypothyroidism, rapid growth spurts, renal osteodystrophy and poor nutrition. It is bilateral in 1/3.
PLAIN FILM
• Early slippage best seen on lateral or frog lateral views.
• Femoral head often slips posteriorly first, then medially.
• Line of Klein (drawn along the lateral aspect of the femoral neck should intersect the lateral aspect of the superior femoral epiphysis).
• Look for a widened physis with indistinct margins or reduced epiphyseal height (due to slipping).
• Ill-defined areas of increased opacification (metaphyseal blanch sign) may be seen in the proximal metaphysis, which is thought to represent a healing response.
