Non-Hodgkin lymphoma is the most common malignancy of the small bowel, often affecting the terminal ileum. Characteristically, lymphoma infiltrates the muscularis and myenteric plexus, leading to large masses with aneurysmal dilatations and no obstruction. If the oesophagus is affected, it is usually due to direct invasion from a gastric tumour.
CT
• Aneurysmal dilatation—large, irregular, cavitatory lesion without obstruction
• Circumferential bowel wall thickening
• Multifocal
• Thickened folds
• Extraluminal mass
• Nodal proliferation and enlargement
Lymphoma. Axial computed tomography image with intravenous contrast demonstrating gross circumferential mural thickening with aneurysmal dilatation of the bowel with mild thickening of further small bowel loops but without proximal obstruction.
