“Rare—there is obstruction of the third part of the duodenum by its compression between the SMA and aorta.
It is associated with severe weight loss (fat around the duodenum is reduced), lumbar lordosis and pregnancy.
Patients present with intermittent abdominal pain and vomiting; this may be relieved when prone.![]()

CT
- Sagittal reconstructions of a CT angiogram are required.
- Look for a tight angle between the SMA and aorta (between 6° and 22°).
- The distance between the aorta and SMA is reduced to 2–8 mm.
- Expect dilatation of the proximal duodenum and stomach.
