These are found in up to 40% of children aged less than 2 years; 95% are seen in those aged under 20 years of age. After 30 years of age, they are rare. They are benign, asymptomatic and regress spontaneously. Lesions >2 cm are known as non-ossifying fibroma. Most affect a single bone, 25% are polyostotic (associated with neurofibromatosis, fibrous dysplasia or Jaffe-Campanacci syndrome).
PLAIN FILM
• Well-defined lytic lesion with a thin sclerotic border in the metaphysis of a long bone.
• 80% lower extremity (particularly around the knee).
• May be expansile.
• Aligned along the axis of the bone.
• If >3.3 cm and occupying more than half of the bone diameter, they should be follow-up.
