Refers to infarction occurring in the diaphysis or metaphysis. Causes include steroid use, sickle cell disease, transplant recipient, trauma, Gaucher disease, etc. Infarction occurring in the epiphysis is known as AVN.
• Typically metadiaphyseal
• Medullary calcification—a serpentine rim of dense sclerosis surrounding a central area calcified to varying degrees
• Sparing of the corticomedullary junction
• Mild remodelling of the bone
MRI
• Low signal on T1, high T2
• Margins enhance with contrast
